Registration window uses default icon in task bar

The registration window does not have a custom icon defined, so it uses the default form icon when viewing in the task bar


Steps to reproduce:

  1. Select Registration from the Settings menu

  2. Hover over the RV taskbar icon

  3. Notice the registration window uses the default windows forms icon


Expected behavior:

  • The RV icon should be used
